(See: Health and wellbeing strategy 2023 to 2028 | Royal Borough of Greenwich (royalgreenwich.gov.uk) We are looking for a strong candidate to support people with Mental Health/Autism/ Neurodiversity/Learning Disability who are inpatients about to be discharged to a community setting to achieve equality of life chances, live as independently as possible and have the right support from mainstream health and care services. Among various other duties, the postholder will have direct responsibility to provide operational commissioning support for adults who draw on mental health or learning disabilities services and to work closely with the learning disability and mental health market. They will support local community MDTs so that hospital discharges are managed in a timely way, giving additional resource to support the receiving provider to maximise the success of the discharge so that people can live in the place they call home. The candidate will work collaboratively with the market and residents, and with colleagues across the system to support the delivery transformational change through linking with other commissioners to ensure commissioning for outcomes and the further integration of health, care, and other forms of support.
