Northwich is the second largest Care Community and has 2 District Nurse Teams although a Care Community approach is fostered promoting close working for both teams. This post will be based with the Victoria Infirmary DN Team based at Verdin House. You will be joining a hardworking and compassionate District Nurse Team, who work together to provide the best care for patients in the Northwich Care Community. As a District Nurse / Case Manager you will work in partnership with our patients and their families to lead in the delivery of person centred, safe and effective care. The successful candidate will require extensive experience in community nursing, have sound clinical knowledge and skills in the management of patients with complex needs, long term conditions and end of life care. You will have a passion for quality patient care and be able to demonstrate ongoing clinical professional development relevant to this post. You will be supported to develop your leadership, organisational and communication skills and how to manage challenging situations in a professional and flexible manner. Although this post is based in Northwich, there will be at times a requirement to work across the footprint of Central Cheshire Integrated Care Partnership.
