The role is full time, predominantly Monday to Friday with some occasional weekend, evening and residential work. We are looking for someone with a minimum of 2 years youth work experience in the NHS, voluntary sector, or community services. The Youth Worker will be supporting young people with long term health conditions such as diabetes and asthma. The focus of the work will be on the young person's social and emotional needs within the children's hospital, adult services and the community. The Youth Worker will work alongside the multi-disciplinary team including Doctors, Specialist Nurses, a variety of different Therapies and Health Play Specialists. You will be offering holistic advice and care to the young people that are being prepared for the move from children's to adult healthcare services. The Youth Workers will deliver this work through one to one, group work, peer support and youth work sessions. The Youth Worker should be self motivated, can use their own initiative and be responsive to the needs of young person in a health setting. The Main Duties Of This Role Are Supporting young people with a long term health condition, both when they are an inpatient and outside of the hospital setting. To provide holistic support for the young person to meet their individual needs. Acting as an advocate to multiple services including hospital, education and employment. This requires good communication skills, active listening skills and to be empathetic towards the young persons needs. Provide support and guidance to the young person during their healthcare transition between children's and adult services. Supporting patients on the wards throughout their treatment whilst they are unwell. The Youth Worker should have the ability to deal with challenging situations in the healthcare setting, have a degree of resilience and to be able to work in clinical areas. Plan and run Youth Club sessions and off site activities, including residential weekends. Being creative, dynamic and self-motivated are desirable attributes of the Youth Worker. Being part of the staff team who support the Youth Forum. Working with the multi-disciplinary team, building relationships and working within your professional boundaries are essential.
