The post holder is responsible for the delivery of practice nursing services to the practice population. Supported by the Nurse Manager, they will deliver care within the boundaries of their role, working within practice protocols, focusing upon supporting patients to be healthy, monitoring of long-term conditions, health prevention and screening activities. The duties will include all tasks normally undertaken by an experienced RN and in addition any roles agreed between the nurse and the doctors as appropriate, having regard to current training. They will work collaboratively with the general practice team to meet the needs of patients, supporting the delivery of practice policies and procedures and will also need to have an understanding of the public health profile and population needs in order to be proactive in ensuring services are met. Carry out annual reviews and management for all long-term conditions, maintaining the annual recall system. Assist in and perform routine tasks related to patient care as directed by senior nursing staff and GPs Following agreed clinical protocols with referral to senior nurses or GPs as appropriate assess, plan, develop, implement and evaluate programmes to promote health and well-being, and prevent adverse effects on health and well-being.
