This role is also suitable for newly qualified Health Visitors as we have an excellent established preceptorship programme to support your consolidation of the SCPHN course. We work with families in one of the most diverse boroughs in London to give children the best possible start in life by providing the most up-to-date evidence-based support. As part of our skill mix team, you will contribute to the delivery of the national Healthy Child programme across Hounslow, working closely with partners, such as GPs, children’s centres and social care providing seamless care to the population of Hounslow. You’ll also have an important role helping to recognise and protect vulnerable children. To work in a public health role in the provision of a comprehensive service to facilitate the improvement and maintenance of the health and wellbeing of children, individuals and families. This will be achieved by applying the key principles of health visiting: To search for health needs. To initiate and contribute to strategies designed to promote and improve health and to encourage individuals to take responsibility for their own health. Work collaboratively with all other agencies to safeguard and promote the welfare of children. The post holder will carry overall responsibility for individuals, families and communities in line with contractual agreements and within their sphere of responsibility. Working in collaboration with members of the Primary health care team, allied health professionals and other key members of statutory and voluntary agencies are key components of this role. Conduct comprehensive health assessments for children and families. Implement evidence-based interventions to enhance child development. Collaborate with multidisciplinary teams to provide holistic care. Utilise innovative strategies to engage with diverse communities. Monitor and evaluate the effectiveness of health promotion initiatives. Develop and maintain effective relationships with partner organisations. Why Join Our Team?
