Main duties of the job This will be apatient facing role working across a number of general practices for one of thePCNs in Barnet, managing long-term conditions; working directly with patientsto assess and treat conditions, as well as promote self-care. As a clinicalpharmacist, you will work within your clinical competencies as part of amulti-disciplinary team to provide expertise in clinical medicines management,provide face to face structured medication reviews, manage long termconditions, management of medicines on transfer of care and systems for saferprescribing, manage repeat prescription authorisations and reauthorisation aswell as acute prescription requests, while addressing both the public healthand social care needs of patients in the GP practice(s) that make up the PCN. About us Barnet Federated GPs CIC (BFG) is a not for profit organisation, putting GPs and healthcare professionals at the forefront of Primary Healthcare provision in Barnet. The Federation is building a large clinical and non clinical team who will work as one in developing Primary Care Services in Barnet for its approximate 425,000 patients and support its Primary Care Networks.
