As a ClinicalPharmacist, you will have an undergraduate degree in pharmacy and registrationwith the General Pharmaceutical Council. You will need to be self-motivated andable to work on their own, have excellent written and verbal communicationskills, experience of influencing others and conducting clinical medicationreviews.
Experience in General Practice would be an advantagebut is not essential if the applicant is clinically up-to-date and understandsthe process of a full medication review in the primary care setting.

Main duties of the jobThis will be apatient facing role working across a number of general practices for one of thePCNs in Barnet, managing long-term conditions; working directly with patientsto assess and treat conditions, as well as promote self-care.
As a clinicalpharmacist, you will work within your clinical competencies as part of amulti-disciplinary team to provide expertise in clinical medicines management,provide face to face structured medication reviews, manage long termconditions, management of medicines on transfer of care and systems for saferprescribing, manage repeat prescription authorisations and reauthorisation aswell as acute prescription requests, while addressing both the public healthand social care needs of patients in the GP practice(s) that make up the PCN.
