The role will provide appropriate information and/or undertake a proportionate assessment and promote a culture of independence and positive outcomes when signposting and Commissioning services. You will obtain an accurate, person centred picture of the outcomes a service user wants to achieve and via discussion with the person, family, carers and other assessors signpost or commission the most appropriate service to achieve desired outcomes, optimising independence as a priority. You will have possession of a recognised qualification in Social Work or other allied to Health or appropriate experience in a Community Care field/ Rehabilitation setting. You will have experience in dealing with complex individual cases and or situations and experience of assessing risk in the Community.
