We are currently recruiting for a SPA Coordinator -

Rhondda Cynon Taf County Borough Council.



Salary:
£22.00 - £23.00 per hour



Hours:
Monday to Friday 10.00am - 2.00pm

Working Schedule: Full-Time Role 6 months with view for extension

Role: Single Point of Access Co-ordinator- The Individual will present a positive image of the Single Point of Access service as part of the Adult Services Information, Advice and Assistance (IAA) service to both public and professional contacts.

The role will provide appropriate information and/or undertake a proportionate assessment and promote a culture of independence and positive outcomes when signposting and

Commissioning services.

You will obtain an accurate, person centred picture of the outcomes a service user wants to achieve and via discussion with the person, family, carers and other assessors signpost or commission the most appropriate service to achieve desired outcomes, optimising independence as a priority.

You will have possession of a recognised qualification in Social Work or other allied to Health or appropriate experience in a Community Care field/ Rehabilitation setting.

You will have experience in dealing with complex individual cases and or situations and experience of assessing risk in the Community.

The post will have a base in, Rhondda Cynon Taf County Borough Council.

Adult Short Term Intervention Ty Elai, Williamstown but offers flexible hybrid working from home, 20 hrs Monday - Friday
