Responsibilities will include: Working along side another Field care coordinator and office based care coordinator carrying out Initial Assessments, reviews and risk assessments in the community Alternate weeks on call, engagement with relevant teams eg - social workers, occupational therapist’s. Completing staff spot checks and supervisions covering the rota in Emergencies or when short staffed. The Person: Experience in delivering domiciliary care to clients in their own homes. Experience in providing high standards of care provision. Diploma in Health and Social Care (or equivalent e.g. NVQ2) or working towards qualification. Highly desirable but not essential as training will be provided. Strives to reach the highest standards in care. Communicates effectively, both verbally and non-verbally. Compassionate, patient, and empathetic. Flexible and approachable with a positive attitude, even under pressure. Great team player. Must have a full UK driving license and vehicle Hours/Shifts Monday to Thursday 9am - 5 pm Friday 9am - 4pm Alternate weeks on call"}
