Responsibilities will include:
- Working along side another Field care coordinator and office based care coordinator carrying out Initial Assessments, reviews and risk assessments in the community
- Alternate weeks on call, engagement with relevant teams eg - social workers, occupational therapist’s.
- Completing staff spot checks and supervisions covering the rota in Emergencies or when short staffed.
The Person:
- Experience in delivering domiciliary care to clients in their own homes.
- Experience in providing high standards of care provision.
- Diploma in Health and Social Care (or equivalent e.g. NVQ2) or working towards qualification. Highly desirable but not essential as training will be provided.
- Strives to reach the highest standards in care.
- Communicates effectively, both verbally and non-verbally. Compassionate, patient, and empathetic.
- Flexible and approachable with a positive attitude, even under pressure.
- Great team player.
- Must have a full UK driving license and vehicle
Hours/Shifts
Monday to Thursday 9am - 5 pm
Friday 9am - 4pm
Alternate weeks on call
