Responsibilities will include:

  • Working along side another Field care coordinator and office based care coordinator carrying out Initial Assessments, reviews and risk assessments in the community
  • Alternate weeks on call, engagement with relevant teams eg - social workers, occupational therapist’s.
  • Completing staff spot checks and supervisions covering the rota in Emergencies or when short staffed.

The Person:

  • Experience in delivering domiciliary care to clients in their own homes.
  • Experience in providing high standards of care provision.
  • Diploma in Health and Social Care (or equivalent e.g. NVQ2) or working towards qualification. Highly desirable but not essential as training will be provided.
  • Strives to reach the highest standards in care.
  • Communicates effectively, both verbally and non-verbally. Compassionate, patient, and empathetic.
  • Flexible and approachable with a positive attitude, even under pressure.
  • Great team player.
  • Must have a full UK driving license and vehicle

Hours/Shifts

Monday to Thursday 9am - 5 pm

Friday 9am - 4pm

Alternate weeks on call
