Main duties of the job The Colorectal Support/Straight to test nurse will be a core member of the Multi-disciplinary Team providing direct and indirect support and advice to colorectal cancer patients and their carers working as a key member of the established CNS team. The Colorectal Support/STT nurse will be the first contact from the hospital the patient receives. Job description Job responsibilities The post holder will act as key worker for colorectal cancer patients across the Trust providing specialist advice, information, support and holistic care to patients and their families from an initial suspected cancer pathway referral, to point of a diagnosis of through treatment, to living with and beyond cancer or to end of life care. Inform and support patients at the prior to and the time of diagnosis and subsequent changes of treatment Undertake suspected cancer pathway triage from GP referrals and complete straight to test assessments enabling patients to be booked directly to appropriate tests Undertake nurse led clinics telephone clinics, inpatient reviews, attend consultant surgical and oncology clinics, treatment visits to chemotherapy and radiotherapy units as needed Participate in the development of the colorectal nursing service. Provide cross site cover where needed The post holder will be expected to attend and actively participate in the colorectal MDM as part of the colorectal CNS team in delivering a seamless service to patients Accept and make appropriate referrals from/to other professional colleagues and groups, as well as external agencies. Check and interpret referrals and take action as clinically indicated. Sensitively handle difficult situations that often arise involving patients, carers and health care professionals. Communicate highly sensitive information - i.e. results of tests and investigations - to patients and carers in a clear and empathetic manner.
