The post holder will contribute to the assessment of service users and help deliver collaborative personalised interventions that are service user, carer and quality focused. Through their interventions they will deliver high level, quality care and evidence-based interventions within a multi-disciplinary team. The post holder will be responsible for delivering and maintaining high standards of clinical care within the wider team. The role focuses upon collaboratively enabling and assisting service users to meet daily health, social care and wellbeing needs, in line with personal recovery goals, and facilitating engagement with mainstream services; and supporting and encouraging service users to achieve independence and reintegrate back into the community.
