The post holder is required to action any missing information in line with referral protocols and referral management service flow charts especially the transfer of care (TOCs) documents. To respond to telephone queries from referring clinicians in acutes/CCGs/HUBS/Primary care about their patients regarding status of referrals To participate in regular audits necessary to monitor effectiveness of service provision and identify data issues across each care pathway. Working both autonomously and as part of the team, the successful candidate will work with in close liaison with the a multi-discipline team comprising of nurse coordinators, therapists, social workers to provide comprehensive admin support to help facilitate discharges from Acute hospital Trusts. The successful candidate will need to be able to commit to weekend working on a rotational basis. The role provides a fantastic opportunity to develop within an ever expanding service working with differing health care professionals and senior team members to help shape service improvements. To act as a Pathway Facilitator within the Transfer of Care Hub working in close liaison with the a multi-discipline team comprising of nurse coordinators, therapists, social workers . The post holder is also expected to access and update a variety of systems in accordance with multi trust patients protocols The post holder will be required to effectively communicate with relevant agencies across the whole system including LUFHT Co-Ordinators, Intermediate Care Teams and Urgent Care Teams to collate and transfer accurate information in differing formats, verbally, in writing and via Teams.
