Working both autonomously and as part of the team, the successful candidate will work with in close liaison with the a multi-discipline team comprising of nurse coordinators, therapists, social workers to provide comprehensive admin support to help facilitate discharges from Acute hospital Trusts. The successful candidate will need to be able to commit to weekend working on a rotational basis.
