North Beverley Medical Centre are looking for a 2 care navigators/administrators to join our team. One post is for 16 hours per week and the second post is for 18 hrs per week. The role involves all aspects of reception/admin duties. The role will require at least one 7:45 am start and one 6pm finish. We are open on a Tuesday evening until 8pm and the successful candidate will be placed on a rota to work this shift (approx 1 in 8). Past applicants need not apply and open for UK residents only Main duties of the job The following are the core responsibilities of the care navigator. There may be, on occasion, a requirement to carry out other tasks; this will be dependent on factors such as workload and staffing levels. a.Process and effectively signpost patients to the appropriate healthcare professional, depending on the presenting condition b.Answer incoming phone calls, transferring calls or dealing with the callers requests appropriately c.Process patient requests for appointments d.Process repeat prescription requests e.Initiate contact with and respond to requests from patients, team members and external agencies f.Enter information on the patient care records g.Photocopy documentation as required h.Data entry of new and temporary registrations and relevant patient information as required i.Manage all queries as necessary in an efficient manner j.Carry out system searches as requested k.Maintain a clean, tidy, effective working area at all times l.Monitor and maintain the reception area and noticeboards m.Support all clinical staff with general tasks as requested n. signposting patients to the appropriate services. We are going over to SYSTEM ONE at the end of June. We are an integral part of the Beverley PCN and work closely with the other practices within the PCN. Ourclinical team consists of 2 GP partners, 3 salaried GP's, a paramedic, an ANP and awell established nursing team.
