Providing end of life care and crisis care under the guidance and support of qualified staff, along with assisting people in the community who have had a fall and other occasional support as required. Supporting the DN teams and other NHS Grampian services. You will be joining a great and established team, learn new skills, will be provided with a uniform as part of their rewarding role and most importantly, will make a real difference to patients and their families in the community. You should have or be willing to work towards a Level 3 SVQ or equivalent and have previous experience in a hospital/community setting. Most importantly, we are looking for people with compassion, empathy and good communication skills, together with the ability to provide End-of-life care and home from hospital care to patients in their own home in a dignified and professional manner. This post is over 7-days per week on a rota basis. A full driving license and access to a car is essential, although a car will be available, you will on occasion may be asked to use your own car. You will have the opportunity to work closely with other Health and Social care members in the community, which could include Community Hospitals and District Nurse Teams. You will also be required to use your discretion and initiative to manage your workload on a daily basis, taking into account any day-to-day potential unexpected changes to need arising from direct patient/client contact.
