Part time - 13.5 hours per week Key Responsibilities  Effectively manage referrals from various care settings.  Coordinate and facilitate smooth discharges from hospital to community care.    Utilize and navigate various care pathways to ensure seamless transitions for patients.    Ensure patients are referred to the appropriate community services for their ongoing treatment and support.    Work within a non-patient facing role in a call centre setting. CWP provides health and care services for local people, including mental health, learning disability, community physical health and all-age disability care – including the provision of three GP surgeries.
