Single Point of Access is seeking an experienced, highly motivated nurse with excellent communication skills to join our team based in Ellesmere Port. This role involves working as a community nurse liaison, where the post holder will play a crucial role in facilitating smooth discharges from hospital into the community.
Part time - 13.5 hours per week
Key Responsibilities
- Effectively manage referrals from various care settings.
- Coordinate and facilitate smooth discharges from hospital to community care.
- Utilize and navigate various care pathways to ensure seamless transitions for patients.
- Ensure patients are referred to the appropriate community services for their ongoing treatment and support.
- Work within a non-patient facing role in a call centre setting.
CWP provides health and care services for local people, including mental health, learning disability, community physical health and all-age disability care – including the provision of three GP surgeries.
