Responsible for the clinical management of a defined caseload comprising of patients with acute, chronic, complex, exceptional care needs and end of life care in a variety of community settings (home, nursery, school, respite) within the Community Children’s Nursing service (CCNS). Ensure that patient health care needs are met through holistic assessment, treatment, management and review in partnership with patients and their families. Provide specialist advice to healthcare professionals, other agencies, carers, clients and relatives. The Community Children’s Nurse role encompasses that of specialist nurse and educator. Support the development of the service to improve practice and work collaboratively across healthcare boundaries, implementing and evaluating evidence based standards, guidelines and policies To support the CCNS Team Manager and deputise, in their absence ensuring effective management of staff and workload.
