Manage efficiently and effectively aspects of the service including clinical assessments, triaging, care planning, clinical supervision, mentoring, assess workload/service activity. A corporate caseload management framework is required Completing the rota for the team ensuring adequate cover & skill mix Assess the demand/needs of the patients and families on caseloads and co ordinate appropriately. Undertake appraisals of team members as directed by the Lead Nurse Support with staff members revalidation as a requirement for practice and maintaining NMC registration. Provide supervision as indicated by SMH supervision policy ie 1:1 sessions with line manager, clinical supervision, group/peer supervision Attend internal and external meetings as representative of St Margarets Hospice Care as directed by the Lead Nurse Community Services. Community Clinical Nurse Specialist in Palliative Care (CNS) job purpose The CNS within the community team is a specialist practitioner with extensive knowledge, skills and competency to manage a caseload of patients with advanced, progressive life limiting disease. The CNS will undertake the holistic assessment, planning and delivery of a consistently high standard of specialist palliative and supportive care in accordance with the hospice policy, procedures and standards. The CNS will establish effective working relationships with wider multi-disciplinary teams and act as a resource for professionals and non- professionals involved in palliative care within the community. We have an exciting opportunity to welcome an experienced Clinical Nurse Specialist to join our Community Nurse Specialist team on a 3 month development basis as Deputy Community Team Lead. It will be based and working within Yeovil and the South Somerset area. You will undertake the holistic assessment, planning and delivery of specialist palliative and supportive care for patients, with advanced, progressive life limiting disease. You will join us on a full-time basis and in return, you will receive a competitive salary of £46,468 per annum plus 30% Unsocial Hours Enhancement on Saturdays and 60% on Sunday & bank holidays.
