Job Purpose
The Deputy Lead Nurse Community Services will fulfil the role specification of the Community Clinical Nurse Specialist in Palliative Care and undertake the additional leadership responsibilities as outlined below. This role is primarily office based undertaking community visits where necessary.
Core to this post is the ability to role model and ensure our core competencies are reflected throughout the team. As Deputy Lead Nurse you will be required to provide a positive approach to change management and demonstrate resilience in leadership. You will also be required to play an active part in the delivery of the Clinical Strategy and hold insight and creativity as additional skills.
Responsibilities
- The Deputy Lead Nurse Community Services role requires working with the CNS team and having a balance of clinical and managerial responsibilities.
- The Deputy Lead Nurse Community Services will support the Lead Nurse Community Services with the day-to-day management of the service, deputising in their absence.
- Manage efficiently and effectively aspects of the service including clinical assessments, triaging, care planning, clinical supervision, mentoring, assess workload/service activity.
- A corporate caseload management framework is required
- Completing the rota for the team ensuring adequate cover & skill mix
- Assess the demand/needs of the patients and families on caseloads and co ordinate appropriately.
- Undertake appraisals of team members as directed by the Lead Nurse
- Support with staff members revalidation as a requirement for practice and maintaining NMC registration.
- Provide supervision as indicated by SMH supervision policy ie 1:1 sessions with line manager, clinical supervision, group/peer supervision
- Attend internal and external meetings as representative of St Margarets Hospice Care as directed by the Lead Nurse Community Services. This will involve cross county working.
- Be a role model to community team providing both clinical and leadership responsibilities.
- Leading and engaging the team through change and development
- Identify areas of learning and utilising training opportunities.
- Keep updated with SMH policies and procedures.
Community Clinical Nurse Specialist in Palliative Care (CNS) job purpose
- The CNS
within the community team is a specialist practitioner with extensive knowledge, skills and competency to manage a caseload of patients with advanced, progressive life limiting disease. - The CNS will undertake the holistic assessment, planning and delivery of a consistently high standard of specialist palliative and supportive care in accordance with the hospice policy, procedures and standards.
- The CNS will establish effective working relationships with wider multi-disciplinary teams and act as a resource for professionals and non- professionals involved in palliative care within the community.
We have an exciting opportunity to welcome an experienced Clinical Nurse Specialist to join our Community Nurse Specialist team on a 3 month development basis as Deputy Community Team Lead. It will be based and working within Yeovil and the South Somerset area.
You will undertake the holistic assessment, planning and delivery of specialist palliative and supportive care for patients, with advanced, progressive life limiting disease.

Working closely with the countywide community team lead, your day to day work will include:

- Assessing the needs of the patients and families on caseloads and co-ordinating appropriately.
