This is a full-time role working 36 hours per week. This role is based at a specialist centre for adults with disabilities. The team work to enrich the lives and opportunities for people with learning and physical disabilities through maximising their personal potential. Key Duties: To develop, monitor, implement and review Personal Care/Support Plans to meet the stated objectives of care plans. Support Manager in implementation of change management. Prioritise and allocate work-plan arrangements and take responsibility for service delivery. Provide a flexible and person centred response to people we support and their families Support the Manager in development and maintenance of joint working arrangements with internal and external stakeholders. Provide effective support to people who use the service through person centred care planning. Facilitate local staff induction Responsibility to communicate effectively with staff Deputise for the manager when required Joint working in partnership with local communities, key stakeholders and supporters Responsibility for budget and payment of external resources e.g. agency workers Review update and apply company Health and Safety policy and procedure. E.g, Medication policy, Safe Handling and Lifting Implement Risk Management Policy Conduct regular Risk Assessments Compliance with standards for security of buildings, contents and confidential information Conduct Service Reviews and assist with Quality Audits Requirements: Health and Social Care Qualification at level 3 or equivalent Understanding of the role of management in a social care setting Knowledge and understanding of relevant legislation If this role would be of interest, please apply with an up to date CV as soon as possible"}
