It is part-time position of twenty three and a half hours. The Social Worker role is to provide a comprehensive service to patients, families and carers to offer pre bereavement support. Our Social Workers work within a multidisciplinary team setting based at the hospice and are also required to work in the community. We offer care to patients and their carers in the Hospice’s In-Patient Unit, through the St Gemma’s community team, and via our day services, which include a Day Hospice and Out-Patient Department.
