Job description Job responsibilities Deliver Advance Care Planning/ReSPECT training and facilitate documentation to ensure best transmission of information within system capabilities Support the design and delivery of personalised care training modules and ReSPECT aimed at identified staff groups and deliver training based on their needs Develop relationships with Primary Care network Leads and staff in order to work with them to identify patients likely to be in the last year of life Educate primary health care staff on best use of Palliative Care registers and meetings Work across organisations to ensure engagement with the project aims Manage information to contribute to reports and demonstrate achievement of Key Performance Indicators Work autonomously within a wider health care team and as a lone practitioner. Work across professional, organisational and system boundaries and proactively develop and sustain new partnerships and networks to influence and improve health outcomes and healthcare delivery systems.
