The integrated team consists of Complex Case Practitioners, District Nurse/ Case Managers, Macmillan, and Therapy teams delivering high quality, efficient and safe care. We are looking for enthusiastic caring and compassionate people who possesses a good clinical skill set. You will also be able to holistically asses patients needs whilst ensuring health promotion and quality improvement to meet the needs of our patients. You must have a good knowledge of modern IT software applications.
