Mid Cheshire Hospitals NHS Foundation Trust is seeking a registered nurse to join their Care Community Team in Winsford as a Community Staff Nurse, delivering care within a 7-day service. The role requires strong communication and interpersonal skills, knowledge of recent NHS changes, and the ability to work independently in a community setting, including wound care clinics. This post requires effective communication and interpersonal skills and the ability to be adaptable, reliable and approachable. The successful applicant will have a sound knowledge of recent changes within the NHS and knowledge of the political drivers to provide care closer to home.
As part of a team you will be responsible for the assessment of nursing care needs, implementation and evaluation of care programmes and the delivery of high quality holistic care within the community setting, and within our wound care clinics. You will be expected to support your peers, the team leader and the organisation in promoting the Trusts values and behaviours.
You will need to be able to work independently, possess good time management and organisational skills and be expected to be innovative and flexible to work in other areas if required to meet service need.

You will also be able to holistically asses patients needs whilst ensuring health promotion and quality improvement to meet the needs of our patients. You must have a good knowledge of modern IT software applications.
