To provide support to Police Pathways practitioners in ensuring all patients referred into the team are cared for in line with Pennine Care NHS Foundation Trust policies and procedures To assist qualified practitioners in assessment, planning, implementing and evaluating assessments of patients under a section 136 To ensure that patients access appropriate services. To provide support and interventions to patients in the HBPoS AFC 27/1/25 JME-220-24 Health Care Support Worker Band 3 To sign post patients to third sector provision as appropriate other community resources such as Citizens Advice Bureau, Peer Support Service and other resources found in the Local Borough Signposting Directory. Provide regular and practical support for patients and their carers’ in managing dignity and independence. Assist patients to gain access to resources required to meet their needs. Provide information on health promotion and support physical health management.
