Skills You will be responsible for carrying out social work duties is a fast paced multi-disciplinary team requiring the highest levels of skills, knowledge and professional expertise. You will ensure the delivery of effective, safe integrated services for adults through the promotion of high standards of practice in assessments and person-centred planning, being creative to meet individual outcomes. The role involves: Assessing, planning and review in line with requirements of the Care Act (2014) and s117 of the Mental Health Act (1983). Taking a strengths based and assessed based approach to support Work with a caseload of individuals in a collaborative way to support in the development of their assessments, recovery plans and structured care plans Signpost to non-statutory services or commission packages of care. Complete high quality assessments of Mental Capacity (in line with Mental Capacity Act 2005) and chair Best Interest Meetings Make safeguarding enquiries in line with s42 of the care Act (2014) ensuring that safeguarding is person centred, outcomes achieved and risk mitigated Comply with SWE Professional Standards and be Registered with Social Work England , Comply with legislation, national and local policies and procedures.
