CRHT operates on a 24 hour, 7 day a week basis. CRHT offers to provide intensive home support to services users as an alternative to hospital admission. CRHT work collaboratively with a variety of services to support service users at home CRHT is based in Broadoak Unit and will cover Liverpool and Sefton areas. The post holder will be expected to work on a rotational shift basis in accordance with service demands, this includes working weekends and night shifts. The post holder will work to assist with the continuity of care for service users within the appropriate care setting. The post holder will work from the community environment. Participate in the implementation of specific programmes of care or therapy to develop and enhance socially appropriate adaptive behaviours under the supervision of qualified staff Participate in regular supervision, engage in reflective practice and be committed to continued development Provide information to enable the effectiveness of care to be evaluated. Ensure appropriate records are maintained under the direction of the qualified practitioner. Contribute to the involvement of Service Users, and carers/relatives in the delivery of care Provide reassurance and support to service users and carers/family Work collaboratively within the Care team to ensure that Service Users needs are met Recognise and respond appropriately to urgent and emergency situations Contribute to the development of services Contribute to the effective and efficient use of resources Help to identify early signs of relapse as described within the care plan and alert the appropriate staff involved in the service users care.
