The Support Time Recovery Worker at Mersey Care NHS Foundation Trust is a band 3 position within the Crisis Resolution Home Treatment (CRHT) team, which provides intensive home support to service users as an alternative to hospital admission. The role involves working on a rotational shift basis, including nights and weekends, to assist in delivering continuity of care within the community. Key responsibilities include facilitating personal care, promoting independence through daily living activities, and supporting service users in recreational, social, and vocational pursuits.
Additionally, the Support Worker will collaborate with the care team to ensure service users' needs are met and participate in the development and evaluation of care plans, while maintaining accurate records under the guidance of qualified staff. The position emphasises promoting social inclusion and the overall well-being of service users and their families. Crisis Resolution Home Treatment team are currently looking to recruit band 3 support workers to the team. CRHT operates on a 24 hour, 7 day a week basis.
CRHT offers to provide intensive home support to services users as an alternative to hospital admission.
CRHT work collaboratively with a variety of services to support service users at home CRHT is based in Broadoak Unit and will cover Liverpool and Sefton areas.
The post holder will be expected to work on a rotational shift basis in accordance with service demands, this includes working weekends and night shifts.
The post holder will work to assist with the continuity of care for service users within the appropriate care setting. The post holder will work from the community environment.
The post holder will provide Support, give Time to an allocated group of service users and aim to promote Recovery predominantly within the community environment

Recognise and respond appropriately to urgent and emergency situations

Help to identify early signs of relapse as described within the care plan and alert the appropriate staff involved in the service users care.
Understand and adhere to Trust policies, procedures and clinical guidelines
Report any concern regarding Service User care to the Qualified Practitioner

Participate in Service User and public involvement activities
