The team responds to individuals whose needs may be complex or require on-going monitoring and support in order to achieve their wellbeing outcomes. The team supports people where short-term services are not appropriate. The team also includes the Review team who complete scheduled reviews to ensure that managed care and support arrangements remain eligible and that outcomes are being achieved. Experience of working with adults is desirable but not essential. About The Role Pay Details: Grade 8 Scp 26-30 £ 36,124-£39,513 Grade 9 Scp 31-35 £40,476-£44,711 Hours of Work: 37 Working Pattern: Monday to Friday Main Place of Work: Canolfan Ty Jenner, Barry The role include undertaking assessments for care and support , CHC assessments and working with individuals whose needs may be complex or require on-going monitoring and support . About You You will need: Experience of working with adults who are vulnerable through illness, disability, mental illness, substance mis-use or age-related frailty. Experience of working with adults living with mental health problems, and within mental health services. Assessment of Adult service users and carers. Understanding of illness, disability, mental illness, substance mis-use and age related frailty and its effects on individuals/carers/family Knowledge of relevant legislation eg Social Services and Well-being (Wales) Act 2014, Mental Health Measure (Wales) 2010, Mental Capacity Act 2005 and Mental Health Act 1983. Knowledge of Safeguarding issues and procedures in relation to Adults A good standard of general education.
