The Vale of Glamorgan Council is seeking a Social Worker to join their Long Term Care Service in Barry. This role involves assessing and supporting individuals with long-term care needs, particularly those requiring ongoing monitoring due to complex situations. About You
You will need:
- Experience of working with adults who are vulnerable through illness, disability, mental illness, substance mis-use or age-related frailty.
- Experience of working with adults living with mental health problems, and within mental health services.
- Assessment of Adult service users and carers.
- Understanding of illness, disability, mental illness, substance mis-use and age related frailty and its effects on individuals/carers/family
- Knowledge of relevant legislation eg Social Services and Well-being (Wales) Act 2, Mental Health Measure (Wales) 2, Mental Capacity Act 2005 and Mental Health Act 1983.
- Knowledge of Safeguarding issues and procedures in relation to Adults
- A good standard of general education.
- A recognised Degree/Diploma or equivalent qualification in Social Work i.e. Degree, DipSW, CQSW etc.
- Registration with Social Care Wales
