The Community Nursing team visit patients in their own home as well as care homes. We offer excellent opportunities for training and development of your clinical skills.The team has experienced Senior District Nurses who will ensure that you receive the necessary support for your personal development.The Community Nursing service works a 7-day week offering a variable working pattern which includes weekends and bank holiday working.You will be supported with a comprehensive induction and training package as needed. Main duties of the job To work as part of the District Nursing Team providing high quality evidence based care to patients in their own home under the direction and supervision of a qualified community nurse. To work without direct supervision providing high quality care to patients in their own homes or in a clinic setting. To identify changes in patients' conditions and care programmes and communicate these to the team leader particularly advising of any circumstances which give rise to concerns in relation to patient care. To practice in a safe confident manner at all times respecting and promoting the principles of health, safety and security at all times. To be able to work as part of a team as well as independently understanding the limitations of the role and recognising when to refer to a registered nurse. To provide support and advice to patients and their carers as appropriate. To support patients to encourage self care and the achievement of independence through rehabilitation programmes. To use appropriately any equipment provided for the use of immobile patients referring to any care plan or risk assessment and competently undertake the recommended moving and handling technique.
