The post holder will be mainly based on Bevan Court 1 The Frailty Service Compromises Of: OPAL (Older Persons Assessment and Liaison Service) This new twenty-four bedded unit is geriatrician led and open 24/7 to provide an overnight stay/short stay area for those patients living with frailty who need further review and treatment. A dedicated Ambulatory unit, which provides rapid Comprehensive Geriatric (CGA) for patients with frailty and frailty syndrome. A 16 bed in patient Frailty Assessment ward, that benefits from daily Consultant Geriatrician leadership along with a dedicated team of nursing, therapy, and social care professionals. A Community Frailty Service for St Helens CCG that provides Comprehensive Geriatric Assessment, post discharge from hospital follow up, and provides ongoing management for frail patients in the community. An Urgent Community Response team providing a 2hr response for and urgent health, social or therapy input. Frailty Virtual Wards providing a hospital at home service for those patients living with frailty in the community (own home or care setting). Post holders will also have an extended induction and training programme, prior to commencement in post. Interview date: 28th October 2024 Main duties of the job To work as part of the Nursing Team undertaking household support tasks and direct care tasks as delegated by the Nurse in Charge. To support the qualified Nursing Staff in the maintenance of high standards of nursing care to patients. To assist in providing an environment within the ward that is conductive to patient recovery and pleasant for patients and their families. To accept responsibility for specifically delegated functions within the ward which the Nurse in Charge feels are appropriate for untrained staff to undertake. To ensure where entries are made in nursing documentation, that these are correct, legible and comprehensive and countersigned by a registered nurse. To understand and adhere to all Trust Policies. To ensure the Health and Safety of patients, relatives and other staff is considered and not prejudiced by the post-holder at any time. To ensure that privacy and dignity of patients is paramount and is not compromised. To ensure that all patient information is confidential and is not disclosed to a third party except with permission of the Nurse in Charge. To report any untoward incidents/occurrences. To ensure that all patient care is delivered with the highest degree of professionalism. To attend specifically fire and lifting and handling lectures annually and ensure appropriate updates are attended once H.C.A. training is complete.
