By working closely with our PCNs and other key partners, including the Local Authority and community Organisations, we help people to stay well and live healthier lives for longer. We operate Federation-led services including Respiratory and Spirometry hubs; Community Cardiology; Community Dermatology; Community Diabetes service; Access E-hub and Enhanced Access and we support PCNs with managing their NWL single offer contract of services and the National Network DES contract, including ARRS roles. The key responsibilities of the role will be to: a) provide SystmOne and other administrative support for community health & wellbeing services; and b) provide project support to the Health Inequalities Team. SystmOne/Administrative Support: Provide essential administrative assistance to patient-facing community roles, including Health and Wellbeing Coaches, Community Health and Wellbeing Workers, and other similar services or pilots under the HI team's remit. This ensures the smooth and efficient operation of these roles in delivering community-based support. Project Support: Assist the HI team with project coordination and delivery, enabling the team to effectively fulfil its mission to address health inequalities through innovation and collaboration in primary care.
