To provide a key role in facilitating the seamless transition from community hospital to patient's own home and transferring the care of the patient to other appropriate services to maximise the patient's independence and quality of life. To provide supervision, facilitate the personal and professional development of junior members of staff, enhancing their skills and knowledge and where appropriate line management.
