Initially on a fixed term contract until the end of March 2025 but will be likely to extend with continuation of ARRS funding. You would be required to work from 2-6 sessions per week across Birkwood Medical Centre and Stirling Medical Centre or Clee Medical Centre. We are looking for 2 candidates in this role. Primary Responsibilities Patient Consultations Chronic Disease Management Prevention Care & Screening Referral & Coordination of Care Health Promotion Public Health & Community Engagement Quality Improvement About us Clinical Duties: Provide face-to-face and telephone consultations for routine and urgent care. Conduct patient home visits (low frequency) Manage patient care in surgery, including investigating and reviewing results, developing care plans, and making referrals to secondary care or other providers. o Issue and sign repeat prescriptions, ensuring they align with national/local guidelines. Administer and handle patient-related queries, test results, referral letters, and other tasks in a timely manner. Record clear, contemporaneous consultation notes using S1 clinical system. Ensure safe, evidence-based, and cost-effective care, in line with Practice disease management protocols and NICE guidance. Independently prescribe medications within your scope of practice. Review and manage patient medications following appropriate policies and local clinical guidelines. Utilise other available clinical and support staff in delivering a holistic and patient centred care. "}
