The Complex Discharge Team provides support for patients who have been identified as needing support from partner organisations for a safe and timely discharge. The role will focus on supporting our patients with discharge plans to avoid admission to hospital or to commence planning of discharge care needs from point of admission and you will be supported by a wider Corporate Discharge Team and Ward Clinical Teams. You will need to demonstrate proactive, enthusiastic, energetic patient centered care planning. Working in collaboration with the multidisciplinary team, the post holder aims to facilitate effective, efficient and safe discharges through admission avoidance in urgent care areas and capture newly admitted patients with complex discharge needs commencing discharge planning through SBAR assessments from the point of admission within Kettering General Hospital. As an enthusiastic and adaptable individual, be able to work proactively whilst championing and promoting #HomeFirst philosophy and supporting the clinical and non-clinical workforce. To have an understanding of the internal escalation process, and how effective discharges can help ensure a proactive and effective organisational response to demand and pressures. Expected to deliver care and services within the strategic objectives of the organisation and promote the principles of excellent discharge planning and delivery.
