The Kettering General Hospital NHS Foundation Trust is seeking a Corporate Discharge Co-ordinator to join their Complex Discharge Team. This role is focused on ensuring safe and timely discharges for patients who require support from partner organisations. Ideal candidates should possess strong people, planning, and organisational skills, and must be adaptable to work a flexible schedule over a seven-day period.

Key responsibilities include developing discharge plans to prevent unnecessary hospital admissions and facilitating discharge planning upon patient admission.
