The service will operate 7 days per week and will provide a rapid assessment hospital admission avoidance for patients with heart failureproviding timely optimisation of evidence based treatment and therapies. This is an exciting post, within a well-established team of specialist nurses. We are looking for highly skilled and experienced administrators withspecialist knowledge and skills who contribute to a culture of continuous quality improvement. The role requires flexibility, administrative skills working within an evolving proactive, dynamic and committed multidisciplinary community Heart failure Team.
