As a Frailty Practitioner, you'll work closely with Advanced Clinical Practitioners (ACPs), trainee ACPs, Consultant Practitioners, Physiotherapists, Occupational Therapists, and Health Care Support Workers to provide timely treatment and care planning following initial assessments by senior team members. What You'll Be Doing *Supporting admission avoidance by delivering treatment and care planning after an initial assessment by senior team members. *Collaborating with the Multidisciplinary Team (MDT) to provide person-centred care. *Delivering interventions at home to include but not limited to holistic frailty reviews placing the person at the centre of care, re-assessing and monitoring treatment plans, point of care testing, IV fluids, IV antibiotics, ECG's *Delegating tasks and supervising junior staff *Working on the Falls Car, alongside SCAS, to assist uninjured fallers and ensure safe recovery at home with a full holistic and functional review. Our service is delivered over 7 days, 8am-8pm. We offer flexible working, but the ideal candidate will be able to work flexibly within these hours including working 1 weekend in 4.
