Here, you’ll make a real difference by providing high-quality, rapid care to patients in the comfort of their own homes, helping to avoid unnecessary hospital admissions and ensuring they receive the support they need.
In this dynamic role, you’ll be working alongside a supportive Multidisciplinary Team, collaborating with experts like Advanced Clinical Practitioners and Occupational Therapists to deliver personalised, person-centred care. Your skills will shine as you conduct holistic frailty reviews, manage treatment plans, and assist individuals in recovering safely at home. With flexible working hours across seven days, including the option to work one weekend in four, we are committed to supporting your work-life balance while you transform lives in your community.
