Keen to be part of our supportive and friendly team Interested in developing yourself and the team you work with Key responsibilities To work with people, including their advocates and carers, to coordinate the identification of their needs. To undertake comprehensive assessments of individuals, including those with diverse or complex presentations, to develop person-centred SMART goals and support plans designed to meet needs in ways that maximises independence, choice and control, and are legal and affordable. To work as part of the multidisciplinary team, participating in the development of services and other staff, providing direction and assistance to support workers, other members of the team and students. Take an active part in your own supervision and appraisal and to supervise and appraise allocated staff in accordance with the departmental supervision policy and councils appraisal policy. To be responsible for your continuous professional development, ensuring your practice is up-to-date and evidence based, attending all mandatory training and specialist training where appropriate. Work in a flexible manner, to provide out of hours cover you will work one late shift a week 12 noon 8pm, and one weekend in 6 ( with time off in lieu over the working week).
