The cancer support worker for Personalised Care Follow Up (PCFU) will form an integral part of the Colorectal multidisciplinary team, alongside the CNS team caring for the physical, social and psychological needs of the patients who have completed their treatment for cancer. The support worker will assist the MDT in coordinating all appointments, signposting patients as necessary to the relevant team member to support the patient through the self-managed part of their pathway. This is part of a project. We will be looking to make this post substantive if successful in June 2025 At the heart of everything we do are our core values: Include, Respect, and Improve. Act as point of contact for queries relating to scheduling of patient appointments and co- ordination of patient follow up pathways. The patients maybe challenging i at times due to the anxiety of their cancer follow up. Respond to patient and other Healthcare professional's communication acting as a triage for patients and ensuring they are signposted to the appropriate member of the team. Manage the database for all patients on the PCFU, liaising with the MDT coordinator to ensure patients are reviewed as necessary at the weekly MDT. Be aware of the physical, emotional and psychological needs of the patient. Able to observe, listen and report any changes to the patient's condition to a member of the MDT so necessary guidance can be given.
