The East and North Herts NHS Trust is seeking a Personalised Care Support Worker for the Colorectal Cancer Team in StevenAGE. This role requires an experienced administrator with strong communication and IT skills to support the implementation of personalised care pathways for patients who have completed their colorectal cancer treatment. The support worker will play a vital role in coordinating appointments, triaging patient inquiries, and ensuring patients are directed to the appropriate team members, while also managing patient data and facilitating holistic needs assessments.

In addition to administrative responsibilities, the support worker will be an integral part of the multidisciplinary team, addressing the physical, emotional, and psychological needs of patients. They will use their observational skills to report any changes in patients' conditions and contribute to the overall care process in line with the trust’s core values: Include, Respect, and Improve. This position is currently part of a project, with the potential for permanency in June 2025, depending on the project's success.
