Responsibilities will include care coordination of service users, Duty Worker on rotation, initial and follow up assessments and supervision. The team is approachable and open. The Community Mental Health Team is focussed on delivering a recovery service for clients with severe and enduring mental health issues. Due to the nature of this post in order to carry out home visits and attend patient meetings a car driver is essential with access to own car. We are interested to hear from you and what new ideas you can bring to the team. Consideration will be given for the right candidate to be supported in development to Band 6 competencies. The Senior Community Mental Health Practitioner will work as a core member of the Community Mental Health team to co-ordinate the care of a complex caseload of service users supervising a Band 5 Community Practitioner/Band 3 Community HCSW where appropriate. Working under the supervision of the Deputy Team Manager/Team Manager the practitioner will formulate care and risk management plans in accordance with identified need and ensure that these are recovery focussed and developed in collaboration with the service user and where appropriate their relative and carer.
