About us Key Responsibilities: Providing leadership and guidance to the team in Hospice Community Services. The post holder will support the teams to ensure the delivery of the right care at the right time to individuals with a palliative care need in the community. The post holder will have a commitment to collaborative working, actively supporting and liaising with other health care professionals and agencies. The post holder will have responsibilities ensuring high quality care is delivered across Hospice Community Services, supporting the patient journey to ensure appropriate and seamless care. To facilitate effective communication within the team and across departments to promote collaborative efforts. To assist in audit and research projects, particularly within the service, taking a lead on specified projects as appropriate.
