St. Luke's Hospice is seeking a motivated Community Nurse Team Leader for their Hospice Community Services in Basildon. This role involves leading a team of Community Nurses to deliver high-quality palliative and end-of-life care to patients and their families in the community. The Team Leader will provide essential guidance and support, ensuring that care is timely and effective while fostering collaboration with other healthcare professionals and agencies.

Key responsibilities include overseeing care standards, facilitating communication within the team, and actively participating in audit and research projects.
