This exciting new post will be hosted by NHS Devon ICB and will support patient flow in and out of both units, in accordance with the Southwest Regional Front Door protocol. The successful postholder will need to demonstrate strong collaborative and system working skills and be able to communicate with responsible clinicians, senior commissioners, social care and other key stakeholders. There is a requirement to work in a matrix and flexible way across multiple areas/teams. The successful postholder will be a key contributor in the operationalisation of the service and will be an integral part of the multidisciplinary team whose remit is to ensure that the right person is supported in the right environment including community or hospital by the right team of professionals, with a clear rationale for admission and robust discharge planning to ensure an appropriate length of stay. To Co-ordinate and failsafe, the patient journey from referral to admission and treatment including long term follow up to ensure timely and effective delivery of patient care in accordance with service specifications, standard operating principles and the southwest regional front door policy. To work with consultants, clinical and non-clinical teams, social care providers, service leads and regional colleagues to improve pathway delivery and inform of community options. To oversee all ICB administrative and clinical processes to enable high quality effective patient focussed service to be delivered. Be an accessible contact point for the person and their support team throughout the pathway. Keeping them updated with accessible information. Enabling them to be actively involved in all aspects of their care, treatment and future plans. To ensure that standard operating procedures are followed ensuring achievement of all performance targets and nominal length of stay.
