The base for this post is in Glossop however the team covers the whole of the North High Peak area. This is a rural location and requires travelling across the locality. Training days are sometimes in other parts of Derbyshire. Thee are various posts with a minimum of 22.5 hours per week. To provide Health Visiting services to families in the community team caseload by delivering the Healthy Child Programme. To manage a defined active caseload and take responsibility for Safeguarding Children issues within that caseload. To have continuing responsibility for the assessment of individuals needs and the development, implementation and evaluation of programmes of care. To prescribe according to assessed individual need from the Nurse Prescribing Formulary. To manage a team of Health Visiting support staff.
